Shopped, I can tell by some of the pixels, having seen a few in my time, the black boxes covering the student name and signature, and the fact that it is from the future.
How is feb. 8th 2012 the future?
In other countries when you write date you first write the day of the month, then the month. You should get some general culture.

I think the teacher had the child fill in their own form ( as a means of reflection on their naughtiness ). Her initials are those at the bottom left. Much more fluid and properly drafted.
I too am a teacher and am too busy to hand out such nonsense but then I teach bigger kids… Cussing and throwing stuff, general bullying and belligerence are what I give detentions for. Oh and being a lazy little defecate.
I do believe that sharing cat pictures and videos should be encouraged and ICANHAZCHEEZBURGERZ is after all 60% of the internetz.
